453.5 - Student Physical Examinations
All children entering the School District of Fort Atkinson are encouraged to have a physical examination.
Physical examinations may be required for athletic participation, for those students with special physical needs, and for those who may require special nursing services.
All new students entering the School District of Fort Atkinson not obtaining a physical examination are requested to complete a Physical Examination Waiver Form.
